• 門診服務時間：上午診 09:30-12:30；下午診 14:00-17:00

• 門診最晚報到時間：門診結束前半小時 12:00/16:30 

• 診所服務時間：週一至週五 09:30-17:30 (12:30-13:30 午休)

• 週末及國定假日休診

• 物理治療復健服務時間：上午 09:30-12:30；下午 13:30-17:30

• 物理治療復健最晚報到時間：治療結束前半小時 ➡️12:00 / 17:00

Physical Therapy Service Hours:

• Morning Session: 09:30 AM - 12:30 PM

• Afternoon Session: 01:30 PM - 05:30 PM

Latest Check-in Time for Physical Therapy:

• Morning session: 12:00 PM

• Afternoon session:  05:00 PM

注意事項：

1. 網路/手機預約服務於看診前28-84天前開放網路掛號

2. 元氣診所聯絡方式：(02) 8071-3615。如需電話預約掛號或是其他服務，櫃檯人員將會為您服務

3. 為確保門診服務品質，上午診最後掛號時間：11:30，下午診最後掛號時間：16:00。病況緊急及經看診醫師同意者不在此限。

4. 各時段門診最後報到時間為門診結束前半小時。上午診最晚報到時間：12:00、下午診最晚報到時間：16:30

5. 門診就醫時就醫時請攜帶健保卡，或其他足以證明身分之文件(如身分證、駕照、護照、有照證件)，嬰幼兒請攜帶戶口名簿及兒童健康手冊。外籍人士請準備護照/居留證及健保卡。若該次未帶健保卡就診，須先以全自費身分批價繳費，並應於三日內持健保卡至診所櫃檯補登及辦理退費。

6. 臨時停診/治療：如遇天然災害或其他不可抗力因素，經行政院人事總處或新北市政府宣布停止上班時，診所即停止門診及復健治療服務。當天所有預約掛號將自動取消，不計缺診，若仍有就醫需求，請重新掛號。若遇醫師或治療師因無法預期之原因臨時請假，我們將盡快通知您並為您改約其他時段

Notice:

1. Online/mobile appointment booking opens 28-84 days in advance of your desired visit.

2. For phone appointments or other inquiries, please contact our clinic at (02) 8071-3615.

3. To ensure the service quality, last registration time for morning clinic is 11:30am, and 4:00pm for afternoon clinic. Emergencies and cases approved by the attending physician are exceptions.

4. The latest check-in time for morning clinic is 12:00pm, and 4:30pm for afternoon clinic.

5. Please bring National Health Insurance (NHI) card or other valid identification (e.g., ID card, driver's license, passport) when visiting the clinic. For infants and young children, please bring their household registration book and child health handbook. Foreigners should bring their passport/residence permit and National Health Insurance card. If you do not bring your National Health Insurance card for the visit, you must pay in full price and apply for a refund at the clinic counter within three(3) days with your NHI card.

6. Temporary closure/treatment: In the event of natural disasters or other force majeure events, if the Directorate-General of Personnel Administration, Executive Yuan, or New Taipei City Government announces a suspension of work, the clinic will suspend outpatient and rehabilitation services. All appointments for that day will be automatically canceled and will not be counted as a missed appointment. If you still need medical care, please make a new appointment. If a doctor or therapist is temporarily absent due to unforeseen circumstances, we will notify you as soon as possible and reschedule your appointment.

7. The appointment time is an estimated time and may vary depending on the actual clinic situation.
